Review clinician notes and data for prior authorization approvals and care plans.
Develop disease- and drug-specific protocols to maximize adherence.
Monitor ongoing patient adherence (PDC, assessments, mobile data) across sites.
Evaluate EMR for drug interactions, labs, allergies, and renewals.
Recommend alternative regimens per guidelines and optimize therapies.
Provide MTM and pharmacist consultations; coordinate care transitions.

🎯 Requirements

PharmD or equivalent and licensed pharmacist.
1+ years clinical/pharmacy experience preferred.
Excellent verbal and written communication.
Deep knowledge of specialty pharmacology and regulations.
EPIC/QS1/WILLOW experience is a plus.
Ability to improve medication adherence and coach patients.
