  • Review clinician notes and assessments to support prior authorization and care plans.
  • Develop disease- and drug-specific clinical protocols to maximize adherence.
  • Monitor adherence and data (PDC, labs, app data) across sites.
  • Review EMR for drug interactions, labs, orders, and MD communication.
  • Recommend alternative regimens per guidelines and disease state as needed.
  • Conduct MTM, document interventions, and assess adherence.

🎯 Requirements

  • PharmD or equivalent; licensed pharmacist.
  • 1+ years of experience preferred but not required.
  • Knowledge of specialty pharmacy domains such as oncology, transplant, HIV.
  • Proficiency with EPIC, QS1, WILLOW; Microsoft Word, Excel, PowerPoint.
  • Strong communication, leadership, and HIPAA compliance.
  • Ability to motivate and coach patients to improve adherence.
