🎯 Requirements
- PharmD required; active pharmacist license in DC, MD, VA, GA, TX
- 5–8+ years of clinical pharmacy experience, ideally in cardiology, ambulatory care, or population health
- Experience with heart failure management and chronic disease management
- Prior experience building or scaling programs
- Demonstrated ability to move from individual contributor to people leader
- Experience in value-based care, population health, or managed care preferred
- Strong operator with a bias toward action, ownership, and continuous improvement
