Recommend guideline-based regimens and disease-specific treatments.
MTM: monitor regimens, document interventions, assess adherence.

🎯 Requirements

PharmD or equivalent; licensed pharmacist in Oregon.
Education: PharmD or equivalent required.
1+ years experience preferred; not required.
Knowledge of complex/chronic disease states in specialty pharmacy.
Proficiency with EMR systems; strong communication; HIPAA compliance.
