Clinical Outreach Specialist

📋 Description

• Targeted patient outreach to close care gaps and support transitions.
• High-touch engagement for high-risk patients: post-discharge follow-up.
• Document interventions and med reconciliation across multiple EHRs.
• Educate patients and assess eligibility for programs with empathy.
• Collaborate with practices to support TCM, OMW, ED follow-ups.
• Meet and sustain performance metrics; other duties as assigned.

🎯 Requirements

• BSN with active, unrestricted RN license (CompactRN preferred)
• 2+ years in case management or discharge planning
• Telephonic outreach and clinical assessments experience
• Strong communication and interpersonal skills
• Experience with EHRs and population health tools
• Independent and interdisciplinary teamwork ability
