Own patient relationships and support care plans.
Hybrid role with field visits in Aroostook County or Presque Isle office.
Perform phlebotomy and collect lab specimens.
Administer immunizations and injections; maintain vaccine records.
Document clinical data in the EHR and assist with care coordination.
Educate patients on treatment plans and follow-up care; support population health efforts.

🎯 Requirements

At least 2 years clinical experience as LPN or CMA
Phlebotomy experience is required
Injections (vaccines) experience preferred
Reliable personal transportation
Strong communication and customer service skills
Ability to multitask with strong organizational skills
