Home Health Nurse- LPN/ RN

Client Summary: We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide overnight care for an 9-year-old boy who suffered an anoxic brain injury at a young age. He is tracheostomy and ventilator-dependent and resides with his loving mother in a smoke-free, pet-free home. His care is administered in a fully equipped, beautiful, apartment-style converted basement. The primary need is for night shift coverage, with flexible 12-hour shifts.

Centria Healthcare works hard to provide in home nursing care for children born with respiratory complications. Shift times vary from 8hrs a day up to 12 hrs a day. We offer flexible nurse driven scheduling. We have part time opportunities as well as full time opportunities. You can even work with Centria as a supplement to your current job. The opportunities are endless.

We are looking for a nurse that is ready to take a step back from the chaotic hospital environment and work one-on-one with a child in their home. Home Health nurses are heroes in Centria's mind. Responsibilities

1. Perform duties outlined in patient's plan of care
2. TBI + SCI rehabilitation
3. Pediatrics / Adults
4. Ventilator and Tracheotomy care (Training available)
5. Gastrostomy tube care
6. Dispensing medications
7. Observe client’s physical, mental and emotional conditions.
8. Report any changes
9. Document observed changes
10. Communicates effectively with Clinical team to increase optimal outcomes
11. Maintains professional nature within the home setting

Required Qualifications, Skills, and Knowledge

1. Excellent oral and written communication skills
2. Ability to follow verbal and written instructions 
3. Ability to utilize computers/tablets and computer software
4. Current state license as an RN or LPN
5. Valid driver’s license and reliable transportation
6. Current negative TB skin test or negative chest X-ray
7. Current flu vaccine
8. CPR certified

Schedule:
