Centralized Liaison (West Coast)

📋 Description

• Provide off-site, full-service medication management and adherence support.
• Off-site clinic team member handling prior authorizations, financial assistance, onboarding, and refill support.
• Educate patients on how the hospital pharmacy improves care.
• Manage medication needs, renewals, and adherence tracking.
• Resolve insurance issues and conduct benefit investigations including PAs.
• Document patient data in EMR, Therigy, and Shields tools; maintain confidentiality.

🎯 Requirements

• Strong interpersonal and telephonic communication; engage patients and coordinate with clinicians/payors.
• Proficiency with Microsoft Excel and Word; quick to learn Shields’ tools.
• Experience with EMR systems (Therigy) and patient documentation.
• Knowledge of prior authorizations and benefit investigations; resolve insurance issues.
• Pharmacy Technician licensure (state-registered); CPhT preferred; 2-3 years’ experience (3-5 preferred).
• High school diploma or GED; organized, detail-oriented, empathetic.
