๐ฏ Requirements
- 2+ years outreach, navigation, and intensive case management for high-risk populations (stabilization, housing, substance use treatment).
- 3-5 years direct case management with high-barrier populations.
- Experience with TAY, justice-involved/reentry, recovery, unhoused, and underserved communities.
- Strong de-escalation, motivational interviewing, overdose reversal, data collection, and case note skills.
- Street-based outreach experience in urban settings and homeless encampments.
- Ability to read, write, collect data, and document case notes to professional standards.
