  • Provide trauma-informed, client-centered case management to diverse caseload (TAY, reentry, unhoused).
  • Conduct psychosocial assessments, needs evaluations, and individualized service plans.
  • Engage clients via one-on-one meetings, outreach, and crisis intervention.
  • Help participants navigate systems: workforce development, education, housing, healthcare, and legal services.
  • Collaborate with job developers, training coordinators, and partners to align goals with employment.
  • Prepare clients for job readiness: resumes, interviews, workplace expectations, and soft skills.

๐ŸŽฏ Requirements

  • 2+ years outreach, navigation, and intensive case management for high-risk populations (stabilization, housing, substance use treatment).
  • 3-5 years direct case management with high-barrier populations.
  • Experience with TAY, justice-involved/reentry, recovery, unhoused, and underserved communities.
  • Strong de-escalation, motivational interviewing, overdose reversal, data collection, and case note skills.
  • Street-based outreach experience in urban settings and homeless encampments.
  • Ability to read, write, collect data, and document case notes to professional standards.
