Case Manager, RN

📋 Description

• Conduct clinic nursing assessments to determine needs and goals.
• Actively participate in IDT meetings to develop care plans.
• Deliver and document nursing interventions per care plans.
• Coordinate with the PACE Physician and IDT to revise care plans.
• Instruct participants and families on disease processes and self-care.

🎯 Requirements

• BSN preferred; CA RN License required.
• BLS and First Aid certifications required.
• CPR certification within 180 days of hire and maintained thereafter.
• 2 years nursing experience with frail/elderly population preferred.
