Conduct nursing assessments and participate in IDT to develop care plans
Deliver and document nursing interventions per care plans; maintain safe environment and meds
Collaborate with the PACE physician and IDT to coordinate care needs and revise plans with participants and families
Instruct participants and families on disease process, self-care and prevention strategies

🎯 Requirements

California RN license (unencumbered)
BSN preferred
BLS/First Aid certification required
2 years nursing with frail elderly preferred
CPR and First Aid certification within 180 days of hire
