Conduct nursing assessments; participate in IDT to develop care plans.
Deliver and document nursing interventions per care plans; administer medications.
Collaborate with IDT and PACE Physician; revise care plans with families.
Instruct participants, families, and caregivers on disease process and self-care.

🎯 Requirements

Graduate of an accredited school of nursing; BSN preferred.
Unencumbered California RN License.
Current BLS/First Aid Certification required.
Two years nursing experience with a frail/elderly population preferred.
CPR and First Aid certification within 180 days of hire; preferred.
