Case Manager, RN

📋 Description

• Lead RN Case Manager guides care as IDT's primary voice
• Own outcomes and quality of care with PCP and IDT
• Conduct PACE nursing assessments at least every 6 months
• Perform urgent and routine follow-up visits (virtual or in-person)
• Develop care plans with IDT, participants, and families
• Manage clinical inbox: orders, results, and actions

🎯 Requirements

• Graduate of accredited school of nursing with unencumbered RN license
• Minimum 2 years nursing experience with frail or elderly population preferred
• Highly motivated, self-directed, adaptable in changing environments and emergencies
• Excellent clinical, organizational, and communication skills with seniors, families, IDT
• CPR and First Aid certification within 180 days of hire (preferred at hire)
