Conduct nursing assessments to determine needs, preferences, and goals.
Actively participate in IDT meetings to develop participant care plans.
Deliver nursing interventions per care plans and document care; maintain a safe environment.
Respond promptly to physician orders and administer medications correctly.
Coordinate with the WelbeHealth PACE Physician and IDT to adjust care as needed.
Instruct participants, families, and caregivers on disease processes, self-care and prevention.

🎯 Requirements

Graduate of accredited nursing program; BSN preferred.
Unencumbered California RN License.
Current BLS/First Aid Certification.
Two years of nursing experience with frail/elderly population preferred.
CPR and First Aid certification by hire; maintain thereafter.
