Conduct clinic nursing assessments to determine participant needs, preferences, and goals
Deliver and document nursing interventions per care plans, including medication administration
Collaborate with WelbeHealth PACE Physician and IDT to coordinate participant care
Revise care plans with participants and families; participate in IDT meetings
Instruct participants and caregivers on disease process, self-care techniques, and prevention
Maintain a safe environment and respond to physician orders

🎯 Requirements

BSN preferred; California RN license (unencumbered)
Unencumbered California RN license
BLS and First Aid certification required
2 years nursing experience with frail/elderly population preferred
CPR and First Aid certification within 180 days of hire (preferred on hire)
