Conduct nursing assessments to determine needs and goals; participate in IDT to develop care plans.
Deliver nursing interventions per care plans; maintain safety; respond to orders; administer meds.
Coordinate with the WelbeHealth IDT and families to revise care plans.
Instruct participants and caregivers on disease process, self-care, and prevention.

🎯 Requirements

Graduate of accredited nursing school; BSN preferred.
Unencumbered California RN license.
Current BLS/First Aid certification.
Two years nursing experience with frail/elderly population preferred.
Must obtain CPR and First Aid within 180 days of hire.
CPR and First Aid certification upon hire preferred.
