  • Conduct nursing assessments to determine needs and goals; participate in IDT meetings to plan care.
  • Deliver and document nursing interventions per care plans; administer medications.
  • Collaborate with the IDT and PACE Physician; revise care plans with families.
  • Instruct participants and families on disease processes, self-care, and prevention.

๐ŸŽฏ Requirements

  • Graduate of an accredited school of nursing; BSN preferred
  • Unencumbered California RN License
  • Current Basic Life Support (BLS)/First Aid Certification required
  • Two years nursing experience with frail/elderly population preferred
  • CPR and First Aid within 180 days of hire; ongoing certification preferred
