- Collaborate with the IDT and PACE Physician; revise care plans with families.
- Instruct participants and families on disease processes, self-care, and prevention.
๐ฏ Requirements
- Graduate of an accredited school of nursing; BSN preferred
- Unencumbered California RN License
- Current Basic Life Support (BLS)/First Aid Certification required
- Two years nursing experience with frail/elderly population preferred
- CPR and First Aid within 180 days of hire; ongoing certification preferred
