Conduct nursing assessments to determine needs and goals; participate in IDT meetings to plan care.
Deliver and document nursing interventions per care plans; administer medications.
Collaborate with the IDT and PACE Physician; revise care plans with families.
Instruct participants and families on disease processes, self-care, and prevention.

🎯 Requirements

Graduate of an accredited school of nursing; BSN preferred
Unencumbered California RN License
Current Basic Life Support (BLS)/First Aid Certification required
Two years nursing experience with frail/elderly population preferred
CPR and First Aid within 180 days of hire; ongoing certification preferred
