Case Manager, RN

📋 Description

• Guide participant care as voice on the interdisciplinary team.
• Perform required PACE assessments and develop care plans.
• Deliver nursing interventions and administer medications.
• Coordinate care and communicate with families to revise plans.
• Mon-Fri schedule; no weekends or on-call.

🎯 Requirements

• BSN preferred; accredited nursing program graduate.
• Unencumbered California RN license.
• BLS and First Aid certification required.
• Two years nursing experience with frail/elderly population preferred.
• CPR and First Aid certification within 180 days of hire.
• CPR and First Aid certification upon hire preferred.
