Conduct nursing assessments to determine needs and goals; participate in IDT to develop care plans
Deliver and document nursing interventions per care plans; administer medications
Collaborate with the WelbeHealth PACE Physician and IDT to coordinate care; revise plans with families
Instruct participants and families on disease process, self-care techniques, and prevention strategies
Consistent Monday-Friday schedule; no weekends or on-call

🎯 Requirements

BSN preferred; accredited nursing program graduate
Unencumbered California RN license
Current BLS and First Aid certification required
2+ years nursing experience with frail/elderly population preferred
CPR and First Aid cert within 180 days of hire; maintain certification
