- Conduct nursing assessments to determine needs and goals with IDT.
- Deliver and document nursing interventions, including med administration.
- Collaborate with the IDT and WelbeHealth physician to coordinate care.
- Instruct participants and families on disease, self-care and prevention.
- Monday–Friday schedule; no weekends or on-call.
- Support transitions of care and ensure a safe environment.
🎯 Requirements
- Graduate of accredited nursing program; BSN preferred.
- Unencumbered California RN license.
- Current BLS/First Aid certification.
- Two years nursing experience with frail/elderly preferred.
- Must obtain CPR and First Aid certification within 180 days.
- CPR and First Aid certification upon hire preferred.
