- Conduct nursing assessments to determine needs and goals; participate in IDT.
- Deliver and document nursing interventions per care plans.
- Collaborate with WelbeHealth PACE Physician and IDT.
- Instruct participants and families on disease, self-care and prevention.
🎯 Requirements
- Graduate of an accredited nursing program; BSN preferred.
- Unencumbered California RN License.
- Current BLS/First Aid Certification required.
- Two years nursing experience with frail/elderly preferred.
