📋 Description
- Collaborate with care team to develop patient-centered care plans
- Ensure timely care progression and safe transitions
- Manage patients across the care continuum and identify barriers
- Coordinate services and adherence to evidence-based pathways
- Review cases within 24-48 hours of admission and daily thereafter
- Partner with providers to develop and execute care plans
🎯 Requirements
- RN license (compact, unrestricted) required; BSN preferred
- 3–5+ years in case management, care coordination, or acute nursing
- Strong clinical judgment and care transitions expertise
- Experience with utilization management and discharge planning
- Excellent communication, organization, and problem-solving skills
