Coordinate care across inpatient, outpatient, post-acute, ER, and home care
Proactively reach out to members with complex health needs and develop care plans
Assist hospital case managers with discharge planning
Educate members by phone or secure messaging about health conditions and medications
Ensure compliance with applicable laws and regulations
Perform other duties as assigned

🎯 Requirements

Active, unrestricted RN licensure in the US or compact multistate license
Ability to obtain additional state licenses to meet business needs
2+ years of clinical experience in payer, hospital, outpatient or community based care management
1+ years of experience in Care Coordination and Navigation
