Provide hands-on personal care and ADLs in participants' homes.
Assist with Instrumental ADLs: laundry, light housekeeping, shopping, and meal prep.
Measure and document participants' vital signs as instructed.
Act as a medical escort to appointments and assist with transfers.
Assist Rehab team by carrying out home exercise programs as indicated in the care plan.
Provide translation services as needed.

🎯 Requirements

High school diploma or equivalency.
Current BLS and First Aid certification required.
CNA or HHA certification preferred.
Bilingual preferred.
Must be registered with Home Care Registry prior to start date.
Must obtain CPR and First Aid within 180 days of hire and maintain certification.
