Provide hands-on personal care and assistance with ADLs in participants' homes.
Assist with Instrumental ADLs: laundry, light housekeeping, shopping, meal prep.
Measure and document participants' vital signs as instructed.
Act as medical escort to appointments and support transfers in/out of homes.
Assist the Rehabilitation team by carrying out home exercise programs per care plan.
Provide translation services as needed.

🎯 Requirements

High school diploma or equivalent
Current BLS/First Aid certification
CNA or HHA certification preferred
Bilingual preferred
Must be registered with Home Care Registry prior to start date
Must obtain CPR and First Aid within 180 days of hire (certification preferred at hire)
