📋 Description

• Provide hands-on personal care and assistance with ADLs at participants' homes.
• Assist with Instrumental ADLs: laundry, light housekeeping, shopping, and light meals.
• Measure and document participants' vital signs as instructed.
• Act as a medical escort to appointments, aiding safe transfers.
• Assist Rehabilitation team with home exercise programs per care plan.
• Provide translation services as needed.

🎯 Requirements

• High school diploma or equivalent.
• Current BLS/First Aid certification required.
• CNA or HHA certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry prior to start.
• Must obtain CPR and First Aid certification within 180 days of hire.
