- Provide hands-on personal care and assistance with ADLs in participants' homes
- Assist with Instrumental ADLs: laundry, light housekeeping, shopping, meals
- Measure and document vital signs as instructed
- Act as medical escort to appointments, aiding safe transfers
- Help Rehab team with home exercise programs per care plan
- Provide translation services as needed
- Maintain a stable, full-time schedule with 2-5 visits per day; 8-hour day
🎯 Requirements
- High school diploma or equivalency
- Current BLS/First Aid certification
- CNA or HHA certification preferred
- Bilingual preferred
- Registered with Home Care Registry prior to start date
- CPR and First Aid certification within 180 days; hire preferred
