- Provide hands-on personal care and assistance with ADLs in participants' homes
- Assist with instrumental ADLs: laundry, light housekeeping, shopping, meals
- Measure and document participants' vital signs as instructed
- Act as a medical escort to appointments, supporting safe transfers
- Assist Rehabilitation team by carrying out home exercise programs per care plan
- Provide translation services as needed
🎯 Requirements
- High school diploma or equivalency
- Current BLS/First Aid Certification
- CNA or HHA certification preferred
- Bilingual preferred
- Must be registered with Home Care Registry prior to start date
- CPR and First Aid certification within 180 days of hire (preferred)
