- Provide hands-on personal care and assistance with ADLs in participants' homes.
- Visit 2–5 patients per day per care plan; ensure an 8-hour workday.
- Reimburse travel time and mileage between participants.
- Act as a medical escort to appointments and assist safe transfers.
- Assist Rehabilitation team with home exercise programs per care plan.
- Provide translation services as needed.
🎯 Requirements
- High school diploma or GED
- Current BLS/First Aid certification
- CNA or HHA certification preferred
- Bilingual preferred
- Must be registered with Home Care Registry prior to start
- CPR and First Aid certification within 180 days of hire (preferred at hire)
