Provide hands-on personal care and assistance with ADLs in participants' homes.
Assist with Instrumental ADLs: laundry, light housekeeping, shopping, meal prep.
Visit 2-5 patients per day based on needs; 8-hour workday.
Measure and document vital signs as instructed.
Act as a medical escort to medical appointments; support safe transfer.
Assist Rehabilitation team by carrying out home exercise programs as indicated.

🎯 Requirements

High school diploma or equivalency.
BLS/First Aid certification required.
CNA or HHA certification preferred.
Bilingual preferred.
Must be registered with Home Care Registry before start date.
CPR and First Aid certification within 180 days of hire; maintain thereafter.
