Provide hands-on personal care and ADLs to participants in their homes per care plan.
Assist with instrumental ADLs: laundry, light housekeeping, shopping, and meal prep.
Measure and document vital signs as instructed.
Act as a medical escort to appointments, assisting safe transfers.
Assist rehab team with home exercise programs per care plan.
Provide translation services as needed.

🎯 Requirements

High school diploma or equivalency
Current BLS/First Aid certification
CNA or HHA certification preferred
Bilingual preferred
Must be registered with Home Care Registry prior to start date
