Provide hands-on personal care and assistance with ADLs in participants' homes
Assist with instrumental ADLs: laundry, light housekeeping, shopping, and meal prep
Measure and document participants’ vital signs as instructed
Act as a medical escort to appointments and assist safe transfers
Help Rehabilitation team carry out home exercise programs per care plan
Provide translation services as needed

🎯 Requirements

High school diploma or equivalency
Current BLS/First Aid Certification required
CNA or Home Health Aide (HHA) certification preferred
Bilingual preferred
Must be registered with Home Care Registry prior to start date
