📋 Description

• Provide hands-on personal care and assistance with ADLs in participants' homes.
• Assist with Instrumental ADLs: laundry, light housekeeping, shopping, meals.
• Measure and document vital signs as instructed.
• Act as a medical escort to appointments, assisting safe transfers.
• Support Rehabilitation team by carrying out home exercise programs as needed.
• Provide translation services as needed.

🎯 Requirements

• High school diploma or equivalent.
• Current BLS/First Aid certification required.
• CNA or HHA certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry prior to start.
• CPR and First Aid within 180 days of hire.
