Care Navigation Nurse

📋 Description

• Deliver coordinated, patient-centered virtual care navigation by phone and online.
• Lead proactive outreach to high-risk members; complete assessments and care plans.
• Utilize Nursing Process and holistic approach including pharmacy interventions and crisis support.
• Assist members in navigating the healthcare system; provide education to meet goals.
• Provide longitudinal care navigation during acute episodes and transitions.
• Identify barriers to health and link to benefits resources and SDOH.

🎯 Requirements

• Active RN license in a compact state with ability to obtain licensure in other states.
• 2+ years of health-plan or TPA case management and 2+ years in hospital or clinical setting.
• Experience across multiple specialties (med/surg, oncology, behavioral health, maternity, critical care, pediatrics).
• Strong competence with EMRs, preferably Salesforce, and Google Workspace.
• Ability to talk, type, and navigate an electronic chart remotely.
• Experience in telephonic care management with no direct/face-to-face contact.
