Care Navigation Nurse

📋 Description

• Coordinate care across inpatient, outpatient, post-acute, ER, and home care.
• Reach out to members facing health challenges; develop care plans.
• Collaborate with hospital case managers to aid discharge planning.
• Educate members by phone or secure messaging on conditions, meds, procedures.
• Ensure compliance with applicable laws and regulations.
• Other duties as assigned.

🎯 Requirements

• Active US RN license or compact multistate license.
• Ability to obtain additional state licenses as needed.
• 3+ years clinical experience in hospital/outpatient/community care.
• 3+ years in Care Coordination and Navigation.
• Bonus: BSN; CCM; Milliman Guidelines; Behavioral health exp.
