Care Manager, Health Homes (Bronx & Manhattan, NY / Field-based)

📋 Description

• Field-based role requiring travel to patients' homes in Bronx/Manhattan.
• Outreach and enroll eligible patients in Health Home Care Management.
• Complete assessments and develop patient-centered care plans.
• Coordinate care with patient's care team and providers.
• Screen health home eligibility and perform initial assessments.
• Maintain patient confidentiality and document services.

🎯 Requirements

• Associate's degree required.
• At least 2 years of Care Management experience.
• Must travel to Bronx and Manhattan.
• Ability to travel to appointments with patients.
• Strong knowledge of local community resources.
• Experience with data reporting and documentation; Salesforce preferred.
