🎯 Requirements
- Bachelor’s degree or social worker license; 2+ years with elders or disabilities
- Valid MA driver’s license with insured vehicle
- Strong communication and assessment skills; relate to patients and families
- Experience with CRM systems (Salesforce) preferred
- Knowledge of local community resources; ability to navigate services
- Travel throughout MA; meet members in person
