Care Integration Manager, Hospice

📋 Description

• Manage transition of complex, chronically ill clients between BAYADA Home Health and Hospice.
• Use data to advocate for and coordinate benefits at the right time.
• Hybrid role with weekly travel to Horsham, PA.
• Educate and engage clients and families about hospice with physicians.
• Coordinate referral documentation and staffing needs across service offices.
• Deliver cross-practice in-services for Home Health and Hospice staff.

🎯 Requirements

• Four years' college degree.
• Five years’ experience in nursing, social work, care coordination, and care integration.
• Excellent verbal and written communication skills.
• Ability to work across settings and practice lines in a matrixed local environment.
• Demonstrated interpersonal and organizational skills.
• Proficiency in Word, Excel, SharePoint, Outlook.
