📋 Description

• Coordinate care across disciplines to ensure best clinical outcomes.
• Address Social Determinants of Health with the care team.
• Assist patients navigating care and accessing resources.
• Lead comprehensive care plans with Nurse Care Manager.
• Facilitate transitions of care across settings and dialysis centers.
• Cross-cover between markets when needed.

🎯 Requirements

• 3 years of healthcare coordination or related field.
• Experience with a team-based approach to care.
• Excellent knowledge of community resources.
• Strong prioritization and multitasking skills.
• Self-directed and motivated to resolve issues professionally.
• Strong communication and teaching skills.
• Intermediate MS Office, including Outlook and Teams.
• Awareness of PHI/HIPAA protections in remote work.
