📋 Description

• Outreach-driven CCM coordination for CKD, hypertension, diabetes, and CVD.
• Create, maintain, and reinforce care plans.
• Coordinate communication between patients, providers, and partners.
• High-volume patient outreach via text and phone.
• Support engagement time standards for Medicare CCM billing.
• HIPAA-compliant documentation and data handling.

🎯 Requirements

• Certified Medical Assistant
• 1–3 years healthcare experience
• Chronic disease or nephrology experience preferred
• Strong clinical assessment, communication, and documentation
• Comfortable in two systems (EHR + CCM)
• Home internet: min 25 Mbps down, 10 Mbps up
