Carbon Health is looking for a Behavioral Health Care Coordinator (BHCC) to serve as a member of a multidisciplinary health care team for the purpose of optimizing mental health. The Behavioral Health Care Coordinator (BHCC) promotes improved access and care management for clinic patients who have behavioral health care needs. This position is an essential member of the primary care team, maintaining strong relationships with primary care providers and behavioral health clinicians, facilitating the integration of care, and coordinating services and follow up with patients and providers.

What You'll Do

  • Accepts referrals of patients between medical and behavioral health providers to facilitate the integration of care.
  • Virtual visits with patients during behavioral health visits to facilitate intake function.
  • Assists in establishing and maintaining smooth functioning and relationships between primary care providers, behavioral health clinicians, primary care consulting psychiatrists, and other clinic departments.
  • Ensures prompt follow up to care by coordinating appointments with patients and care providers.
  • Follow up with patients via phone or other forms of communication with regards to the patient’s capacity to follow through with recommended care plans.
  • Tracks client attendance, calls patients with day-before-appointment reminders, follows up on no-shows, and addresses any barriers to care to support and improve continuity of treatment.
  • Refers to clients who are in crisis or requiring a higher level of care need to appropriate mental health services providers.
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Document patient progress in EHR and other required systems so as to be shared with medical providers, psychiatric consultants, and other treating providers.
  • Complete comprehensive psychosocial assessment.
  • Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
  • Participate in regularly scheduled caseload consultation with the psychiatric consultant, present clinical information effectively and efficiently.
  • About You

  • Experience or training working in a Collaborative Care (CoCM, BHI) model is a plus.
  • Have experience with integrated health care systems.
  • Committed to evidence-based practices and data-driven treatment.
  • Understand HIPAA requirements for mental health records and maintain appropriate documentation in compliance with legal and ethical standards.
  • Ability to work in compliance with policies and procedures while working within a collaborative framework.
  • Proficient computer and technology skills.
  • Excellent communication skills both verbal and written; including expertise with presentation skills.
  • Qualifications

  • Associate or Bachelor’s Degree in Health, Human Services, or Social Services Field for at least 2 years
  • Expertise in handling higher-acuity patients/situations
  • Technical proficiency in a variety of platforms including EHRs, ability to quickly learn new technology
  • Knowledge of/experience with behavioral health screening tools (PHQ9, GAD7, CSSRS, etc.)
