Currently, we provide virtual and in-person mental health services, including comprehensive evaluations, medication- management and therapy, across Pennsylvania and Virginia with plans to expand to new markets in 2025.

Position Summary:

Blackbird Health is seeking a Clinical Partnerships Coordinator  to strengthen collaboration with our most important community partners—pediatricians, schools, and mental health providers—through accurate reporting, seamless information sharing, and proactive communication.

This role is focused on ensuring that partners have the timely, clinically relevant updates and support they need to coordinate effectively with us. You’ll manage reporting processes, troubleshoot issues, oversee the transfer of documentation, and highlight key clinical cases that require attention. Your work will ensure that partnerships run smoothly and that providers see Blackbird as a trusted, responsive collaborator.

The ideal candidate is detail-oriented, proactive, and comfortable working across clinical and non-clinical settings. They’re data-comfortable, solutions-focused, and skilled at building trust through clear, consistent communication.

ESSENTIAL FUNCTIONS:

• Design, manage, and execute clinical reporting processes for top referring partners.
• Provide regular status updates and reporting on shared patients’ care progress.
• Troubleshoot clinical reporting and documentation issues, working cross-functionally to resolve them quickly.
• Oversee the secure transfer of reports, clinical documentation, and other required records to community partners.
• Collaborate with Blackbird’s clinical teams to identify key cases or updates to proactively surface to partners.
• Support the market development team in other outreach activities to create new referring partner relationships and further cultivate existing ones. 
• Act as a trusted representative of Blackbird, ensuring all communication reflects our values and commitment to care.

OUR IDEAL CANDIDATE WOULD HAVE:

• A strong personal connection to the Blackbird mission.
• A Bachelor’s Degree (required), preferably in Healthcare Administration, Nursing, Business, Education, or a related field.
• At least three (3) years of relevant experience in healthcare, education, or partner/client relationship management.
• Strong communication and interpersonal skills, with the ability to maintain trusted relationships across organizations.
• Comfort working with clinical data and reports, including spreadsheets and documentation systems.
• Exceptional time management and organizational skills.
• Ability to identify and solve problems proactively in collaboration with both clinical and non-clinical teams.
• Self-motivation, flexibility, and the ability to manage multiple priorities simultaneously.
• Strong computer skills and proficiency in Google Suite.
