Behavioral Health Care Manager (BHCM)

📋 Description

• BHCM on Cerula Care\'s collaborative care team; coordinate with psychiatrists and coaches.
• Conduct initial biopsychosocial intake and safety screens for cancer patients.
• Arrange follow-ups, brief interventions, and telehealth communications.
• Document progress in HIPAA-compliant EMR and patient registry.
• Lead weekly interdisciplinary meetings and document psychiatric recommendations.
• Serve as care coordinator, liaising with referring specialists and the team.

🎯 Requirements

• Active, unrestricted clinical license in at least one US state (e.g., LCSW, LICSW, LPC).
• Bachelor’s degree in nursing, social work, or health/human services; Master’s preferred.
• Two+ years of clinical care management experience with cancer/advanced illness preferred.
• Must be bilingual in English and Spanish.
• Telehealth engagement and strong care coordination skills.
• Experience with HIPAA-compliant EMR systems.
