- Serve as the primary contact for patients in the Collaborative Care program.

🎯 Requirements
- Bachelor's degree in social work or related behavioral health field.
- Training in brief psychosocial interventions and motivational interviewing.
- 2+ years in behavioral health, integrated care, or care management.
- Licensure such as LCSW/LMHC/LMFT/Psychologist or LMSW trainee preferred.
- Strong collaboration and teamwork skills.
- Experience with screening for mental health and substance use disorders.
