• Make home visits to clients in designated territories.
\n
• Conduct evaluations for speech, language, communication, or swallowing difficulties.
\n
• Develop and update treatment plans based on evaluation, orders, and goals.
\n
• Continuously assess and revise care plans, and participate with BAYADA clinicians in the multidisciplinary care plan, as appropriate.
\n
• Educate and instruct patients, family, or caregivers in rehabilitative care and safety.
\n
• Accurately document observations, interventions, and evaluations in EMR using a touchpad tablet.
