Making home visits to clients in designated geographic territories.
Administering medication, wound care, treatments, and comfort care.
Monitoring clients' conditions and reporting changes to Clinical or Client Services Manager.
Following doctors' orders and properly documenting them.
Performing client assessments as necessary.
Occasional on-call after hours and weekends.

🎯 Requirements

A current license as an RN in VT and NH.
A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
Prior home care and/or hospice experience strongly preferred, but not required.
Solid computer skills; EMR experience preferred.
Ability to travel to assigned cases.
