Visit clients in their homes to support recovery after injury, illness, or surgery.
Provide 1:1 direct care in client homes.
Develop care plans and manage cases; deliver skilled care (assessments, meds, wound care).
Support patients from discharge toward home goals.
Educate patients and families about care and procedures.

🎯 Requirements

Active and current RN license
One year of clinical experience preferred
Previous home health care experience welcome, but not required; we will train you
Ability to travel locally to cases as assigned
