Visit clients in their own homes to support recovery after illness or surgery.
Case manage, develop care plans, with assessments and wound care.
Progress clients from discharge to meeting goals at home.
Provide education to the patient and/or their family.
Collaborate with a 24/7 clinical support team as part of an interdisciplinary care team.

🎯 Requirements

Active and current RN license
One year of clinical experience preferred
Previous home health care experience required
Ability to travel locally to cases as assigned
