- Rotating on-call schedule 5pm–8:30am (7 on/7 off)
- In-person home health visits in Gloucester, Salem & Cumberland, NJ
- Triage patient calls and perform home visits as needed
- Follow designated care plans and document changes
- Be part of a multidisciplinary care team delivering home health
🎯 Requirements
- NJ RN license
- 1+ year clinical (medical/surgical) experience
- Home care experience preferred
- Graduation from accredited nursing program
- Ability to work independently and manage time
- Strong interpersonal skills
- Proficient with EMR; EMR experience preferred
- Ability to travel to cases as assigned
