- Conduct RN visits in Senior Living communities in Charlottesville, VA area.
- Follow designated care plans and provide 1:1 patient care.
- Perform home visits and deliver medications, wound care, and treatments.
- Monitor patient conditions and report changes to Clinical/Client Services Manager.
- Document observations and interventions in EMR/tablet; ensure proper records.
- Coordinate with Clinical Team for continuity of care and case management.
🎯 Requirements
- Active Virginia RN license.
- Minimum 1 year of recent clinical (medical/surgical) experience.
- Home health experience preferred but not required.
- Graduation from an accredited nursing program (transcript/diploma).
- Ability to work independently and manage time effectively.
- Solid computer skills; EMR experience preferred.
